Sick Leave Bank

To request one or more days of Sick Leave from the Sick Leave Bank, please click on the thumbnail version of the fillable form found below and download a copy to your computer. The employee should complete the top half of the form and return it to benefits@sfps.k12.nm.us along with the employee’s physician’s name and email address. Human Resources will then send the form to the employee’s physician with instructions to complete the bottom half of the form and return it to Human Resources. 
Note: To request one or more days of Sick Leave from the Sick Leave Bank, you must be a member. Any employee receiving district benefits is eligible to join the Sick Leave Bank. This can be done at the time of initial hire or during the annual Open Enrollment Period that occurs in September each year. Learn more by clicking on the Procedures form below.
